Initial Evaluation

Your first visit will be an evaluation. We offer this in person and online. Evaluations consist of comprehensive assessments, treatment, and an individualized plan of care for any sport or orthopedic injury.

Duration: 60 minutes

Whether you're recovering from surgery, managing a nagging injury, or preparing for a return to sport, Paradigm offers rehab that goes beyond the basics. We provide individualized, goal-oriented programs designed to help you recover, rebuild, and return to doing what you love — stronger and more resilient than before.

Meet Sharice

Doctor of Physical Therapy

Hi, I'm Sharice - movement nerd, & owner of Paradigm. I earned my Doctor of Physical Therapy degree from the University of St. Augustine for Health Sciences in 2023. Before pursuing this passion, I received my Bachelor's degree in Kinesiology from Iowa State University. I also worked as a certified personal trainer on campus and actively participated in intramurals (mostly sand volleyball!). Since graduating, I've focused on enhancing my expertise in sports rehabilitation, injury risk reduction, and bridging the gap between rehab and performance.

 

I've worked with athletes at every level - youth sports, collegiate competitors, pro athletes, weekend warriors, and everyday gym goers. I focus on bringing high-level experience to a more personal, 1:1 setting, specifically tailored to each individual.

This approach meets the unique needs of each client, combining evidence-based practices with understanding performance demands and recovery processes. Outside of being a physical therapist, I enjoy lifting, walking my dogs, gardening, and exploring new places.

Common Questions

What payment methods do you accept? Most patients opt to pay via card, cash, or Venmo. As a cash-based provider, Paradigm does not work directly with insurance companies. I’ll always provide you with an invoice that you can easily submit to your insurance for potential reimbursement.

Why cash based / out of network rehab? Each year, insurance companies reduce reimbursement rates & become more strict in dictating how clinicians treat patients. With an out of network model, we are able to have full autonomy over the time spent with you, provide HIGH QUALITY care, & lower your healthcare costs all while helping you become stronger & more resilient.

Do I need a doctor's referral? Nope! In the state of Minnesota, you can work with a physical therapist for 90 days without a referral. This allows you to schedule an appointment directly. If it is determined that your condition requires a physician's evaluation, we will refer you.

What to expect in a session? Every visit will be a 1:1 session with your Doctor of Physical Therapy. Your first visit will be a 60 minute initial evaluation consisting of a review of your current pain/injury, past medical history, movement screening, tests & measures, manual therapy techniques, & personalized programming for your rehab game plan. Follow up appointments may consist of a combination of re-assessments, manual therapy, & adjusting exercise prescription. Please plan to wear active/athleisure clothes! There are restrooms & shower rooms available in the facility you can use to change as needed.

How long are sessions? Sessions can be scheduled for 30 or 60 minute time slots.

How long will I have to do PT for? The duration of physical therapy can vary based on symptoms, goals, & individual needs. We'll discuss realistic timelines, expectations, & game plans during our sessions. With that being said - we start to see physiological changes as a response to progressive exercise after 6-8 weeks. Extended timelines are common for post-operative care.

Can I receive services if I'm on Medicare? Yes! As an out-of-network provider, we do not bill Medicare directly. Instead - we focus on offering personalized wellness and performance-based rehab on a self-pay basis. This allows us to provide you with high-quality care without the restrictions of traditional insurance models.

Do you provide online rehab options? Yes! at Paradigm, we offer online rehab session for individuals who aren’t local or prefer the convenience of online care. This is done via video call or through online programming— so you can keep progressing, no matter where you're located. Just select the “online evaluation or follow-up” options when booking, and you’ll receive a secure video link for your session.
